Who I am
I am Jamal Chehab, a licensed life insurance agent, licensed in Alabama, Kentucky, Maryland, Michigan, North Carolina, Ohio, Pennsylvania, South Carolina, Tennessee, Texas and Virginia. I can only help residents of these states. This website is used to receive requests for a life insurance conversation and a free needs analysis (the free Will Kit is provided at the end of the needs analysis). What I collect
When you submit the form, I may collect:
- Name, mobile phone number and optional email
- State, best time to reach you, and whether your interest is family, business or both
- Whether you asked for a free needs analysis and Will Kit
- Any optional message you write
- Your consent choice and the version of the consent text shown to you
- Source (for example a
?src=link parameter), page URL, submission time and approximate IP address (when available)
Why I collect it and how it is used
I use this information to contact you about life insurance, including by call, text or email as allowed by the consent you give, to schedule your needs analysis, answer your questions, and follow up on your request. I do not use it for unrelated purposes.
Where it is stored and who can see it
When you submit the form, your request is sent to me by email from your own email app (you review it and press Send), or it may be stored in a Google Sheet that I control (through Google Apps Script). Access is limited to me and anyone I expressly authorize to help handle requests. Google’s processing of Sheet data is subject to Google’s terms for those services. To record your IP address for consent records, the page asks a free public service (ipify.org) for it; no other tracking or advertising tools are used on this website.
I do not sell your information
I never sell your information. I do not rent or trade your contact details to third-party lead buyers.
How to opt out or request deletion
You may revoke contact consent or ask me to delete the information you submitted by calling or texting 440-541-9270. For text messages, you may also reply STOP to opt out or HELP for help. I will honor opt-out and deletion requests as required by applicable law.
